The Cricket Report in association with Clydesdale and Yorkshire Bank is your chance to put forward your views on the state of the game and what you love and hate about cricket.

Every time you complete one of the surveys, you’ll be entered into the Cricket prize draw, with fantastic prizes up for grabs over the course of the season. The more surveys you complete, the more chance you’ve got of winning. Simples.

The sixth and final part of this year’s survey comprises 12 questions about cricket in 2012 and the state of the English game, and you’ve got until September 3 to complete a survey and get your name in the hat for our star prize – a VIP day for two at Lord’s for the Clydesdale Bank 40 Final on September 15 and a night at the Royal Garden Hotel.
